This chapter discusses the different symptoms arising from progressive respiratory muscle weakness and dyspnoea that happens to ALS patients. This progressive loss of respiratory muscle function in ALS patients ultimately results in respiratory failure, causing death in majority of amyotrophic lateral sclerosis patients. In this chapter, the available management strategies for progressive respiratory muscle weakness are also introduced and discussed. These management strategies include the use of tracheostomy (TV) and non-invasive positive pressure ventilation (NIPPV) wherein both their pros and cons as respiratory management strategies are discussed and analyzed.Less

Dyspnoea generally occurs when the load on the respiratory muscle pump exceeds its capacity. It also occurs when respiratory muscle weakness, abnormalities in the respiratory muscle control, and irregularities in the skeletal muscle occur. The chapter begins with a discussion on the respiratory muscle in healthy individuals and then examines its contribution to breathlessness in disease. Respiratory muscle functions that may contribute to breathlessness include respiratory muscle weakness, respiratory movement disorder, COPD, and chronic heart failure (CHF). The chapter also provides a brief discussion on respiratory muscle function in cancer patients.Less

It explains that dysfunction of respiratory muscles occurs in chronic obstructive pulmonary disease (COPD) patients and that this contributes to the development of dyspnoea. Respiratory muscles can be trained by general exercise training to improve strength and endurance. Inspiratory muscle training (IMT) has been found to be effective in reducing dyspnoea and patients can also benefit from education to change breathing patterns.Less
