This is the fifth book in an international, multi-contributed series aimed at providing practical, clinical guidance on how to deal with difficult symptoms related to specific cancer sites. In it, the editors bring together palliative care and oncological treatment for patients with head and neck cancer. Head and neck cancer is one of the most frightening and distressing cancers for patients and their families as it affects appearance, the ability to speak, and the ability to eat. Pain, which is difficult to treat, infection, and disfiguring surgery with wounds that often do not heal, are common accompaniments of advanced disease. In addition, psychological distress, loneliness, and isolation are often experienced by patients. Head and neck cancer is not that common but, when it does occur, it very often needs specialist help from palliative care and hospice clinicians. There is little written on this subject and this book provides a practical guide that draws together all the information in an easily accessible format. The Palliative Care Consultations series is primarily aimed at those individuals working in an acute hospital cancer centre and/or tertiary referral centre. The books are designed to give the busy clinician advice on clinical problems, both those rarely encountered and those that are very common but difficult.Less

This chapter discusses head and neck cancer with particular attention on patients with carcinomas arising from the upper aerodigestive tract. Head and neck cancers present particular challenges to healthcare professionals as a result of several disease-related factors, treatment-related factors, and patient-related factors. For patients afflicted with such disease, prognosis is relatively low. The poor prognosis is associated with the progression of the malignant disease and the development of associated disease. The first section of the present chapter discusses the principles of management of head and neck cancer while the second section focuses on the palliative care of patients with advanced head and neck cancer.Less

This chapter focuses on malignancy found in the head and neck regions. Head and neck cancers account for five per cent of those diagnosed in Western nations and cause 2,500 deaths in the United Kingdom. Head and neck cancers can be cured at the onset by radical local treatment, while in an advanced progressive stage they pose a great challenge in symptom management. The majority of head and neck tumours are squamous carcinomas arising from the changes in the epithelium of the aerodigestive tract. Lifestyle factors, such as smoking and drinking, and chronic trauma, such as ill-fitting dentures and broken teeth, contribute to their development. Discussed in this chapter is the pathology and patterns of spread of the squamous carcinoma and the management and treatment of head and neck cancers.Less

It provides the natural history of head and neck cancer, and the effect of treatment on morbidity and the health-related quality of life. The chapter also determines whether surgical rehabilitation is appropriate for patients following tumour ablation. Symptom control, surgical palliative procedures, nutritional support, and loco-regional control are discussed as well. The dental considerations in general palliative care are provided in the latter part of the chapter. The chapter determines that palliative surgery for patients who have head and neck cancer is challenging.
