This chapter summarizes the conclusions and recommendations from the Task Force on interventions to reduce the burden of diabetes. For interventions in the healthcare system, the Task Force recommends both disease management and case management. To improve diabetes self-management education (DSME), the Task Force recommends delivering DSME in community gathering places for adults with type 2 diabetes and in the home for adolescents with type 1 diabetes.Less

This chapter demonstrates the breadth and “power” of selected diabetes policies. It discusses ones that are relatively simple which could be directly implemented within a clinic without great difficulty. The chapter talks about policies of such potential impact that they could ultimately influence all people with diabetes in a large metropolitan area, in a country such as the United States, or even throughout the world. Policies of such potential impact are, not surprisingly, both challenging and controversial.Less

The current technology for self-monitoring of blood glucose (SMBG) levels has been well established since the 1980s. This practice is beneficial to patients with diabetes from both a clinical and an economic standpoint. Knowledge of their blood glucose levels can allow patients to select appropriate doses of insulin to regulate these levels. This chapter analyzes the technology, benefits, economic aspects, problems, and controversies associated with SMBG, continuous glucose monitoring, and the artificial pancreas as well as telemonitoring technology for diabetes management.Less

Self-determination theory is grounded in the belief that people work best and are happiest when they feel that they are in control of their own lives. This book explains the ramifications of the theory and provides clinical examples to show that it can be used to motivate patients undergoing treatment for such physical or psychological issues as diabetes management, smoking cessation, post-traumatic stress, obsessive-compulsive disorder, and depression. The first part of the book provides historical background to self-determination theory, showing that it is humanistically oriented and has three decades of empirical research behind it. In the process, the book discusses why humanistic psychology fell out of favor in academic psychology; why “self-help” and New Age books have such perennial popularity; and why it is so important for authorities to support patients' sense of self. The remainder of the book presents many specific case examples to describe the theory's application.Less
