Cities in Karnataka, as in India, suffer from inadequate data and information, which has undermined their ability and that of analysts and policymakers to comprehend the complex forces shaping cities and to develop and implement effective urban policies. Given the importance of cities in Karnataka’s economic growth and development, the book undertakes a review of 15 major cities. While it is clear that no existing studies present the state of cities in India’s context, not collecting this information has disastrous consequences for cities, since they would not be in a position to understand their own growth nor predict their future planning for public services. This book attempts to fill in this vacuum. In this book, indicators and benchmarks are developed for six thematic areas that capture the dynamics and potential of urban areas: history and governance, demographics, economic dimensions, infrastructure and public services, resources, and quality of life. Policymakers, city officials, investors, real estate developers, infrastructure agencies, financiers, industry, credit rating agencies, the educated general public, and researchers would be interested in the book since it has implications for the business environment and quality of living in these cities. The book also provides a description of best practices in service delivery across the 15 cities, so that these can be shared across the cities, and competition encouraged among them for firms, investment, and better residents with promise of a better quality of living.Less

High rates of poverty and homelessness, especially among women and children; increasing alcohol and drug abuse problems; escalating teen violence; and inadequate services for the chronically mentally ill are just a few of the problems that human service professionals encounter. Unfortunately, traditional approaches and programs often fail these vulnerable populations, particularly since their multiple needs often require a multidimensional approach. New innovations in practice and service delivery are clearly needed, but the current literature is often idiosyncratic, fragmented, and does not offer a systematic approach to the issues and problems. This book, containing chapters written by leading scholars in social work and related fields in mental health and human services, presents an interdisciplinary approach to the examination of innovations in direct practice and service delivery, synthesizing the development and application of knowledge concerning practice and delivery across both problems and populations. It allows innovations in direct practice and service which have relevance across problem areas or groups to be shared by students and practitioners dealing with a range of problems across the lifespan. The book is divided into four parts, beginning with a conceptual overview of theory developed by the editors for the analysis of innovations in practice and delivery. The remaining sections focus on children and adolescents, adults and the elderly. Featuring community-based and state-of-the-art services for youth with severe emotional and behavioral disorders and their families, this book describes each intervention in depth, along with the supporting evidence for its utility. Most chapters present a single intervention as an alternative to institutional care. Shared characteristics of these interventions include delivery of services in the community (homes, schools, and neighborhoods) provided largely by parents and paraprofessional staff. The interventions are appropriate to use in any of the child human services sectors and have been developed in the field with real-world child and family clients. In addition, they offer a reduced cost in comparison to institutional care. Several chapters address diagnostic-specific psychosocial and psychopharmacological treatments, which are likely to be provided as adjunctive treatment in a clinical setting.Less

This book provides guidelines for the care of the dying based on the Liverpool Integrated Care Pathway for the Dying Patient (LCP). Developed at a hospice, the information can be disseminated and adapted to fit different settings such as hospitals and nursing homes. The LCP is a multiprofessional document that incorporates evidence-based practice and appropriate guidelines related to care of the dying. It provides a template that describes the process of care, which is generally delivered in a clinical situation and incorporates the expected outcome of care delivery. The LCP replaces all other documentation in this phase of care. Care pathways can provide a potentially powerful aid to professionals involved in palliative care. Basic principles of treatment are translated into daily practice, including bedside documentation systems, policies and procedures, standards of practice, continuing education, and quality improvement programmes. The book also includes chapters on symptom control, ethical issues, communication skills, and spiritual care.Less

It then discusses integrated and coordinated health care delivery systems. It is argued that America could move rapidly to integrated systems if it “opens the markets and levels the playing field” by adopting a reform model like the Wyden–Bennett Healthy Americans Act, the Dutch model, or the one proposed by the Committee for Economic Development (CED), in which everyone would have wide, responsible, individual, and informed choice, and multiple choices among alternative health care financing and delivery systems. Experience shows that when offered such a choice, great majorities of people choose integrated systems. The principles of such reforms are comparatively easy to state. The implementation is, unfortunately, extremely complex.Less

This review aims to identify the most appropriate and cost-effective models of service delivery and level of provision for palliative care services. This includes the role of nurses in hospitals and the community, and the care of dying patients in acute hospitals. Ten key research questions were identified within this aim including the current need for and provision of services, proposed alternative models of provision, appropriate skill mix in nursing, the relationship between alternative models of care and patient satisfaction, carer satisfaction and patient's quality of life, and the impact of alternative models on other services. In this study a comprehensive review of literatures from the UK, North America, Europe, and Australia was conducted. Over 2,000 documents were identified, of which 673 were deemed to be relevant. Data extracted from these literatures were collated and synthesized in the sections of this study, along with tables of summary data derived from relevant papers.Less

Huge care and maintenance programmes were established in nearby asylum countries under the auspices of the fifth High Commissioner, Poul Hartling. The global refugee total tripled and the UNHCR's budgets and programmes quintupled. The emphasis on aid delivery also reflected Western governments’ desire to assist refugee warrior communities fighting Soviet‐backed regimes, resulting in most refugee problems in regions of superpower conflict becoming protracted. At the same time asylum crises arose in Western Europe and North America.Less

This chapter considers the integration of (i) physician services and (ii) the institutional and economic interests of hospitals. These issues provide a vehicle for addressing the institutional fragmentation arising from existing hospital-governance regulation. How, and to what extent, should economic considerations factor into medical-care decision-making? What regulatory structure should govern physician–hospital interrelationships? Whereas hospitals once followed an “eBay” model, they now have their own institutional interests. Hospitals have independent duties to patients, and responsibilities to assure quality of care and manage costs. These realities call into question the traditional regulatory/governance structure of hospitals. The regulatory regime for integrated delivery networks (IDNs) is more flexible than the traditional JCAHO hospital model. The chapter concludes that (i) the legal/regulatory environment should not dictate how physician–hospital relationships should develop, nor the appropriate organizational form; and (ii) regulatory flexibility and “regulatory neutrality” should guide public policy in this arena.Less

It first observes that despite paying equal insurance premiums and enjoying equal insurance coverage, lower-income and non-white workers consume fewer insurance benefits than their white and higher-income coworkers. It finds no evidence, however, that this disparity leads to adverse health outcomes. It additionally finds that non-whites and low-income individuals are more likely than their white and high-income counterparts to obtain mental health care from general practitioners rather than mental healthcare providers, and nearly twice as likely not to follow up with a mental health provider after hospitalization with a mental health diagnosis. These findings suggest that low-income and non-white individuals might be paying for health services that primarily benefit their white and more affluent coworkers. Many of these regressive consequences can be attributed to mental health insurance carve-outs, which are a product of the fragmented delivery of health care.Less

This chapter addresses the past four decades of American health care politics, asking whether the idea of fragmentation provides a central and helpful analytical perspective. Part I reviews the highlights of health policy disputes from the 1970s to the 2000s. Part II places the United States in comparative perspective. Part III discusses the regulation versus competition debate that has been such a dominant part of American medical care discussion since the 1970s. Part IV discusses the issues raised by efforts to reform Medicare, and questions whether the theme of fragmentation illuminates those policy disputes. Part V concludes that fragmentation refers to many disparate features of American medical care and politics, and is illuminating only when tied to specific institutions, internests, and ideas.Less

The relationship of food availability and prices to individual entitlements is studied. The argument then moves to the role of transactions, both interregional (private trade) and intertemporal (hoarding), on famine vulnerability and how the government could intervene in each case. The last part discusses the merits and limitations of cash support, and recommends its greater use.Less

Extroverted and intensely energetic, especially in the fast outer movements, these works display a wide tonal range, an enriched harmonic syntax, fast tempos, and streamlined surface activity. Whereas slow movements feature expressive soloistic embellishment (notably the ternary variation design of Op. 54/3, the gypsy lament of Op. 54/2, and the concerto-style Op. 55/1), the finales concentrate on compositional intrigue — especially Op. 55/1, with its synthesis of fugue (looking back to Op. 20) and rondo (as in Op. 33), and Op. 54/2, famous for the incomparably witty inspiration of a form that thwarts expectations at virtually every turn. Chromatic harmony figures prominently through inflections within phrases and remote tonal excursions within themes.Less

What counts is what works – but how can we actually tell what works? And what can we do with such knowledge to influence policy and practice? As all parts of the public sector embrace ‘evidence’ as a means of providing more effective and efficient public services, this book provides a contribution to such debates. The authors consider the role of evidence in specific public policy areas (healthcare, education, criminal justice, social care, welfare, housing, transport, and urban renewal), using experts in each field to explore the creation, dissemination, and use of evidence within each. They consider in particular: How is research evidence of service effectiveness created? How does such evidence shape policy and influence service delivery? What efforts are being made to encourage greater utilisation of evidence in policy and practice? The rich cross-sectoral accounts of the many and diverse activities in each sector provide an insight into the ebb and flow of evidence as guidance to policy and practice. The book develops perceptive analyses of outstanding problems, and raises challenging agendas for service development and future research. There exists a general consensus for the need of ‘early warning systems’ for policy makers' prompt response to impending famines. But since famines occur for a variety of reasons and a famine's influence on the economic system is varied, a formal early warning is difficult. This chapter argues that famines develop from initial shocks and the underlying dynamic process can help to devise an early warning system. Past data on famine-prone regions can be used as a basis to build appropriate warning systems. An early warning system should be accompanied by a policy response system that considers factors such as institutional structure, national and international politics, and the recent history of the area.Less

Problems lie in the decision as to what to count as religious arguments and how accurately our texts represent what was actually said.Less
