A 36-year-old woman was admitted to the acute medical unit with symptoms of sweatiness, palpitations, and anxiety. She was normally fit and well and had given birth to twins while in India two years ago. She was not taking any regular medication. Ten days previously she had suffered from a flu-like illness with joint pain, muscle pain and fatigue. Subsequently, she developed pain and swelling in her neck and swallowing was uncomfortable. On examination she had clammy palms and a mild tremor. Her temperature was 38.8°C; her pulse was 110 beats per minute and regular. She had a moderately sized, tender goitre. No lymph nodes were palpable. She had lid lag and lid retraction but no ophthalmoplegia or chemosis.
